Population health surveillance has benefitted from informatics analysis and solutions. Health informatics technology provides the data collection tools to support population health management and reporting (Denisco & Barker, 2016). Informatics registries have been successful at not just collecting and storing data, but also at connecting care gap reports with the ability to alert patients that need to be seen (Pramilaa, 2013). Without an automated way of notifying patients, people can fall through the cracks.
